Telephonic Care Coach

Job Description:

Manage members' health issues using telephonic strategies, approaches, and techniques
Identify and resolve barriers hindering effective care
Conduct comprehensive assessments and identify member needs
Guide members and families to essential resources for optimal care
Continuously monitor patient care using assessments, data, conversations with members, and active care planning
Ensure patients progress toward desired outcomes
Follow professional concepts, standards, regulations, strategies, operating standards, and established procedures
Make decisions regarding work approach and priorities
Report to the Manager, Care Management
Occasionally travel to Humana offices for training or meetings

Requirements:

Bachelor's degree in a health-related field, active Illinois LPN licensure, or the ability to obtain IL LPN license
2 or more years of case management, medical case management, care coordination, or healthcare support experience
Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook
Previous experience with electronic case note documentation
Experience documenting in multiple computer applications/systems
Experience with health promotion, coaching and wellness
Knowledge of community health and social service agencies and additional community resources
Ability to work Monday–Friday during operating hours of 8:00 AM–7:00 PM CST, with core hours primarily 8:00 AM–5:00 PM CST
Ability to meet home internet requirements of at least 25 Mbps download and 10 Mbps upload
Ability to work from a dedicated space without ongoing interruptions to protect PHI/HIPAA information
