This role focuses on delivering pharmacist-led clinical services that help patients better manage chronic conditions, improve medication adherence, and achieve positive health outcomes.<br/><br/>The successful candidate will work remotely while collaborating with providers, care coordinators, and interdisciplinary teams to deliver evidence-based, patient-centered care through innovative telehealth programs.<br/><br/>Primary Responsibilities<br/><ul><li>Conduct comprehensive medication reviews and medication reconciliation services</li><li>Identify, assess, and resolve medication-related problems</li><li>Provide telephonic and virtual patient consultations</li><li>Support patients with chronic conditions including diabetes, hypertension, heart failure, COPD, and other complex disease states</li><li>Develop individualized medication management recommendations for providers and patients</li><li>Promote medication adherence through patient coaching and motivational interviewing</li><li>Participate in care transition and post-discharge medication review activities</li><li>Document clinical encounters accurately and in accordance with regulatory and organizational standards</li><li>Collaborate with physicians, nurse practitioners, and care teams to optimize patient outcomes</li><li>Contribute to quality improvement initiatives and patient engagement efforts</li></ul><br/><br/>Minimum Qualifications<br/><ul><li>Doctor of Pharmacy (PharmD) degree from an accredited institution</li><li>Active New Jersey Pharmacist License in good standing</li><li>Minimum of two (2) years of pharmacist experience in ambulatory care, clinical pharmacy, managed care, telehealth, or related settings</li><li>Strong computer proficiency and ability to navigate multiple clinical systems</li><li>Ability to work independently in a remote environment while maintaining productivity and quality standards</li><li>Willingness to obtain additional pharmacist licenses as business needs evolve</li><li>Ability to educate patients in Spanish language</li></ul><br/><br/>Preferred Experience<br/><ul><li>Board Certification (BCPS, BCGP, CDCES, or similar)</li><li>Medication Therapy Management (MTM)</li><li>Chronic Care Management (CCM)</li><li>Transitions of Care (TCM)</li><li>Remote Patient Monitoring (RPM)</li><li>Population health or value-based care programs</li><li>Patient engagement and health coaching</li></ul><br/><br/>What Makes You Successful<br/><br/>Strong clinical assessment and critical-thinking skills<br/><br/>Excellent written and verbal communication abilities<br/><br/>Ability to establish rapport and trust with patients remotely<br/><br/>Highly organized with strong attention to detail<br/><br/>Commitment to quality, compliance, and patient-centered care<br/><br/>Adaptability in a fast-paced virtual healthcare environment<br/><br/>Apply Today<br/><br/>If you are passionate about leveraging your clinical expertise to improve patient outcomes through innovative virtual care models, we encourage you to apply and become part of our mission-driven team.</div>
