Registered Nurse, Telehealth

Providing virtual nursing services, the full-time Registered Nurse, Telehealth will conduct telehealth visits, perform clinical assessments, and offer member education for individuals with chronic cardiometabolic conditions in a remote capacity.

Key responsibilities

• Conduct scheduled telehealth nursing visits and clinical assessments for members managing chronic conditions
• Provide nursing support and education through member messaging, addressing questions related to symptoms and care plans
• Document clinical encounters accurately in the electronic health record and communicate any clinical escalations to the appropriate providers

Required qualifications

• Active, unrestricted Registered Nurse license in good standing, with a Nurse Licensure Compact (NLC) multistate license preferred
• Bachelor of Science in Nursing (BSN) is required
• A minimum of five years of clinical nursing experience, with at least one year in a telehealth or virtual care environment
• Experience supporting individuals with diabetes, obesity, hypertension, or dyslipidemia is strongly preferred
• Proficiency with electronic health records and technology supporting virtual member care
