Job Description:

Serve as the first point of contact for patients through onboarding, scheduling, and relationship-focused outreach
Facilitate CHE completion in nephrology offices, clinics, or via telehealth
Coordinate with Nurse Practitioners and clinics to prepare for in-person appointments
Make outbound calls to patients, caregivers, and clinics for CHE scheduling, reminders, and rescheduling
Schedule telehealth CHEs for patients unable to attend in person
Welcome new patients and collect concerns from existing patients
Escalate concerns to RNs or Nurse Practitioners as appropriate
Maintain accurate documentation and communicate patient updates to the care team
Build trust, influence patient participation, and overcome engagement barriers
Organize outreach by urgency and importance to maximize patient impact
Function across both ES and CC capacities

Requirements:

High School Diploma or GED required
Associate’s or Bachelor’s Degree preferred
Prior IKC experience required (CC/ES)
2+ years professional work experience in a customer service, patient-facing, or telephonic outreach role preferred
Prior experience in a healthcare setting preferred
Intermediate proficiency in Microsoft Word, Excel, PowerPoint, and Outlook
Strong verbal and written communication skills
Active listening and motivational techniques
Highly organized, with excellent time management and follow-through skills
Ability to work autonomously and collaboratively with care team members
Must reside in Pacific or Mountain time zones
Quiet, private home workspace with high-speed internet required
